Before Smile Transformation

Our patient, whom we will refer to as Mrs. T for privacy reasons, had been suffering from years of excessive teeth grinding, a condition medically known as bruxism. This had led to missing teeth and a significant loss of tooth structure in the occlusal (biting surface) and anterior (front) regions of her mouth. This not only affected her appearance but also her ability to chew and speak comfortably. The loss of tooth structure had also caused a change in her vertical dimension of occlusion (the distance between the upper and lower jaws when the teeth are in contact), leading to further functional and aesthetic challenges. Mrs. T was determined to make a change and turned to us at Toronto Smile - Design Yorkville Dental for help.

Digital Smile Design Planning of the Smile Transformation

The Treatment Plan

Dr. Johnson developed a comprehensive treatment plan tailored to Mrs. T's unique needs. The treatment plan was divided into several phases to ensure optimal results and patient comfort.

Phase 1: Assessment and Planning

The first step involved a thorough assessment of Mrs. T's oral health, including a detailed examination of her teeth, gums, and jaw joints. Dr. Johnson also took radiographs (X-rays) and digital photographs of Mrs. T's teeth and face to assess the extent of the damage and plan the treatment accurately. A detailed discussion with Mrs. T about her goals and expectations was also an essential part of this phase.

Digital dentistry refers to the use of digital technologies or devices to carry out dental procedures. These technologies include the use of computer-aided design/computer-aided manufacturing (CAD/CAM) systems, intraoral cameras, digital radiographs, and even software for diagnosing and planning treatments.

Advantages of Digital Dentistry:

  1. Improved Efficiency: Digital dentistry streamlines the treatment process, reducing the time it takes to diagnose and treat patients. For example, digital impressions can be taken in minutes, eliminating the need for traditional molds, which can be uncomfortable and time-consuming.

  2. Increased Accuracy: Digital technology allows for more precise and accurate measurements, resulting in better fitting restorations and appliances.

  3. Enhanced Visualization: Digital imaging allows both the dentist and the patient to have a better view of the teeth and surrounding structures, aiding in diagnosis and treatment planning.

  4. Less Discomfort: Procedures that utilize digital technology, such as digital impressions, are often less invasive and more comfortable for the patient.

  5. Better Patient Education: Digital images and simulations can help patients understand their treatment options better, aiding in informed decision-making.

Digital Smile Design (DSD)

Digital Smile Design is a comprehensive approach to designing a new smile using digital technology. It involves taking digital photographs and videos of the patient's face and mouth from various angles and using software to manipulate the images to show the patient what their new smile could look like.

Advantages of Digital Smile Design:

  1. Personalized Treatment: DSD allows the dentist to create a smile that is tailored to the patient's unique facial features and aesthetic desires.

  2. Visual Communication: It provides a visual tool that helps the dentist and patient communicate more effectively about the desired outcome.

  3. Predictable Results: By visualizing the end result before starting the treatment, the dentist can plan the treatment more accurately, leading to more predictable results.

  4. Increased Patient Satisfaction: Patients who can see a preview of their new smile before starting treatment are often more satisfied with the results.

Mock-Up Process

The mock-up process involves creating a physical model of the patient's mouth, which can be used to test the planned restorations or appliances before they are finalized. This can be done using traditional methods, such as wax-ups, or digital methods, such as 3D printing.

Advantages of the Mock-Up Process:

  1. Risk-Free Trial: It allows the patient to 'try on' their new smile before any permanent changes are made to their teeth.

  2. Improved Communication: The mock-up helps the patient and dentist to have a clear and shared understanding of the treatment goals.

  3. Predictable Outcome: The mock-up serves as a blueprint for the final restorations or appliances, helping to ensure a predictable outcome.

  4. Increased Patient Confidence: Seeing and trying on the mock-up can increase the patient's confidence in the treatment plan and the final result.

In conclusion, digital dentistry, Digital Smile Design, and the mock-up process all play a crucial role in ensuring a predictable outcome for the patient. They allow for more accurate diagnosis and treatment planning, improved communication between the dentist and patient, and increased patient satisfaction with the final result. Together, they contribute to a more streamlined and successful treatment process.

Osseointegration is a fundamental part of the dental implant process at Toronto Smile Design - Yorkville Dental, overseen by Dr. Johnson. This process involves the jawbone fusing with the titanium post of the dental implant, which is critical for several reasons:

  1. Stability: Successful osseointegration ensures that the implant is securely anchored to the jawbone. This stable foundation is essential for the artificial tooth (crown) that will be attached later, allowing it to withstand the forces of biting and chewing.

  2. Longevity: Proper osseointegration increases the likelihood that the implant will last a long time, possibly a lifetime. This reduces the need for future replacements or adjustments, making it a cost-effective and long-term solution for missing teeth.

  3. Natural Feel: A well-integrated implant feels and functions just like a natural tooth. This means you can eat, speak, and smile with confidence, without worrying about the implant moving or causing discomfort.

Time Required for Osseointegration

The time required for osseointegration can vary from person to person, depending on several factors such as the individual's bone density, overall health, and whether bone grafting is required. Generally, it takes about 3-6 months for the jawbone to fully fuse with the implant post. During this time, it is essential to follow Dr. Johnson's instructions carefully and maintain good oral hygiene to ensure successful osseointegration.

In summary, osseointegration is a crucial process that ensures the stability, longevity, and natural feel of dental implants. Dr. Johnson and the team at Toronto Smile Design - Yorkville Dental are experts in this field, providing personalized care and guidance throughout the entire dental implant process.

CAD/CAM technology in dentistry stands for Computer-Aided Design/Computer-Aided Manufacturing. It involves the use of computers to design dental appliances and to control the manufacturing process. This technology has revolutionized the field of dentistry by improving the efficiency, accuracy, and effectiveness of dental treatments. Here are some advantages of CAD/CAM technology when compared to conventional laboratory techniques:

  1. Speed: CAD/CAM technology significantly reduces the time required to produce dental appliances and restorations. While conventional laboratory techniques may take several days or even weeks to complete, CAD/CAM can often produce restorations in a single appointment.

  2. Accuracy: CAD/CAM technology allows for highly accurate and precise measurements and restorations. The digital impressions created using CAD/CAM are more accurate than traditional molds, leading to better fitting restorations and appliances.

  3. Consistency: CAD/CAM technology produces consistent results as it eliminates the human error associated with conventional laboratory techniques. This ensures that each restoration or appliance is of the highest quality.

  4. Customization: CAD/CAM technology allows for a high level of customization. Dentists can design restorations and appliances that are tailored to the patient's unique anatomy and needs.

  5. Material Options: CAD/CAM technology allows for the use of a wider range of materials, including ceramics, resins, and metals. This means that dentists can choose the best material for each individual case.

  6. Less Invasive: The digital impressions used in CAD/CAM technology are less invasive and more comfortable for the patient than traditional molds.

In summary, CAD/CAM technology offers several advantages over conventional laboratory techniques, including speed, accuracy, consistency, customization, a wider range of material options, and improved patient comfort. These advantages make it a valuable tool for modern dental practices.

Transformation of the patient's smile

Phase 2: Preparing the Foundation

The next step involved preparing the foundation for the final restorations. This phase included implant placements, crown lengthening, and temporary crowns for healing. Implant placements involved surgically inserting titanium screws into the jawbone to serve as artificial tooth roots for the missing teeth. Crown lengthening involved surgically reshaping the gum tissue and bone around the teeth to expose more of the natural tooth structure, making it possible to place restorations on the teeth. Temporary crowns were placed to protect the teeth and gums during the healing phase.

Phase 3: Healing

A 3-4 month healing period followed the foundation preparation phase to ensure optimal results. During this time, Mrs. T was advised to follow a soft diet, practice good oral hygiene, and avoid any activities that could potentially damage the temporary restorations or the surgical sites.

Phase 4: Final Impressions and Laboratory Work

After the healing phase, final impressions of Mrs. T's teeth were taken using a high-precision digital scanner. These digital impressions were then sent to our state-of-the-art dental laboratory, where our talented dental technicians meticulously crafted the final restorations over 14 days. The restorations included dental veneers, crowns, onlays, and implant-supported crowns. Dental veneers are thin shells of porcelain that are bonded to the front surface of the teeth to improve their appearance.

Due to our detailed planning, there was no need for adjustments in the final restorations! They were inserted immediately under a microscope with Rubber Dam isolation. This phase was crucial as it involved the placement of the final restorations, transforming Mrs. T's smile both functionally and aesthetically.

One Roof, Multiple Skills

What sets us apart is that all procedures were performed under one roof at Toronto Smile - Design Yorkville Dental by Dr. Johnson. Mrs. T did not need to visit multiple dentists and specialists for a multidisciplinary approach, as Dr. Johnson has both the surgical and prosthodontics skills to complete the case by himself, aided by our super talented dental technicians. This not only ensured continuity of care but also saved Mrs. T valuable time and effort.

The result was a beautiful, functional smile and a renewed sense of confidence. Mrs. T's journey was not just about restoring her teeth; it was about reclaiming her confidence and rediscovering the joy in everyday life. Her happiness was infectious, and it reminded us once again why we do what we do.
