Mrs. D. came to my Cosmetic Dentistry clinic with worn-out dentition and missing teeth in her mouth because she has been clenching for decades. Bruxism was causing devastating damage to her natural teeth. As a Cosmetic Dentist, bruxist patients are the most complex patients to treat. But unfortunately, the bruxism issue is a common issue in society. That's why we need to know the treatment procedure of patients who have bruxism issues. The ultimate goal for us shouldn't be providing a nightguard to mask patients' symptoms. This protection appliance must be the last step.

The intention should be reprogramming the bite to its ideal position called Centric Occlusion with special appliances. After achieving the stability between centric relation and centric occlusion, we need to restore the teeth to their original morphology.

However, smile design must the first step of every restorative treatment plan. With the Digital Smile Design protocol, I obtained photo-video documentation and took impressions using the face-bow transfer. After designing the smile makeover with Digital Dentistry tools, I transferred the design to my dental lab. Later, they transferred my smile transformation design to Digital Wax-up. After the transfer, they printed the 3D digital wax-up using 3D printers. With this 3D printed model, I could transfer my design to the patient's mouth and face. As dentists, we call this dental application mock-up.

I think mock-up is the most crucial step for establishing an understanding between patient and cosmetic dentistry treatment team. It is so important because before we start, we can visualize the outcome of our treatment plan in the patient's mouth. If our patients like it, we continue with the treatment. If the patient doesn't like it, they can walk away or adjust the design according to their needs and desires.

Our patients usually don't expect this kind of change on their faces and look. When the patients see this change, they can visualize what we are talking about. When we talk about dental implants, dental crowns, cantilever bridges, or Maryland bridges, they get bored because it is nearly impossible for them to imagine any cosmetic dentistry application outcome.

It was time to explain the things we need to do to make this test-drive experience a reality. Then, it was time for the treatment plan presentation.
The first treatment needed is splint treatment for achieving occlusal harmony. While working on that, I plan a crown lengthening procedure on anterior teeth to restore the golden proportions. There are some teeth extractions (teeth removal) to be completed. After achieving occlusal equilibrium, we are going to switch to the prosthodontic phase of the treatment.

I didn't want to provide Dental Implants but because of her heavy smoking habit. So I informed her about the increased risks in heavy smoker patients. She requested Dental Bridge instead.

In the front teeth, I provide all-ceramic crowns instead of porcelain veneers to achieve anterior guidance. On the molar areas, we have different types of dental bridges to offer. Instead of traditional bridges, I want to use another type of bridge - multiple porcelain fused-to-zirconia bridges. We will use them to open the bite to give her what she lost from her teeth structure over the years.
