Mr. L. came to the clinic with multiple concerns, such as discoloured teeth, one brown tooth and one yellow tooth in the center of his teeth. He was also concerned about his gummy smile issue. He had a midline shift and gaps between his teeth. He had asymmetry in his teeth; his left side and right side had different smile curves. He came for a cosmetic dentistry consultation. He desired to have the perfect smile he hasn't had for years.

As a cosmetic dentist, I approach the situation in a holistic dentistry approach. I don't want to focus on one issue more than the others. I like the smile transformation to be completed fully. That's why I love to test drive my smile transformation cases whenever I start a case. I design everything on my Mac and share the design with the team members involved in the treatment. For example, if the patient needs orthodontic treatment, I share his digital smile design files with my orthodontist. Also, after constructing the solid and implementable treatment plan, I send my design files and impressions to my lab team to 3D design / 3D print the so-called "smile prototype."

First of all, we should state the obvious, brown teeth and yellow teeth in front teeth areas are significant issues for smile makeover treatments. There are multiple reasons for a patient to have tooth discolorations, like trauma, root canal treatment, failed bonding applications, poor dental hygiene, chewing tobacco, early medication use etc. In his case, the issue was trauma and following ancient root canal treatment techniques.

With this level of internally stained teeth, our treatment options narrow down. For example, I couldn't provide porcelain veneers because of the level of internal staining of the teeth. Porcelain veneers are super-thin and can easily show yellow and brown stains. Internal bleaching could be an option, but we couldn't resolve the issue perfectly. We would still see the shadow of these discoloured and stained teeth under.
